Most first-visit paperwork is covered by three forms. Each is available as a PDF you can print at home and bring with you. Our team is also happy to walk you through any of them at reception — no rush, no jargon.

New Patient Intake

Demographic and contact information, emergency contact, reason for your visit, and a brief minor-medical summary (family doctor, allergies, current medications). The standard first-visit paperwork.

Insurance & CDCP Details

Private insurance details and CDCP enrollment information so our team can set up direct billing before your visit. If you're eligible for CDCP but not enrolled yet, our team can help you apply.

Medical History

Comprehensive health questionnaire — conditions, medications, allergies, surgeries, lifestyle, and dental history. For privacy reasons this form stays PDF-only. Print, fill, and bring sealed to your visit.

New Patient Intake — Online

Demographic, contact, and minor-medical info. Takes about 10 minutes. If you'd rather print and fill by hand, the PDF is up above. All information is handled per Ontario's PHIPA.

When each form is used

Forms by stage of your care

Different forms apply at different points. Here's when each one comes up.

Before your first visit

We email or text the digital new-patient intake on request — contact details, dental history, medical history, insurance, and emergency contact. Takes 10 to 15 minutes from home.

At your first appointment

If you didn't complete the intake in advance, you fill it out at reception before being called in. The reception team helps with any questions. Bring medication bottles or a pharmacy printout if you're not sure of dosages.

Each return visit

A short medical history update form confirms whether anything has changed since your last appointment — new medications, conditions, hospitalizations, or allergies. Takes 2 to 3 minutes.

Before any sedation case

Sedation consent (IV sedation or GA) is reviewed in detail with the sedation provider or anesthesiologist before the procedure. Covers risks, pre-op fasting, recovery expectations, and post-op restrictions.

CDCP application (if needed)

For patients who think they're eligible but aren't yet enrolled, our team helps with the application. Bring your most recent CRA Notice of Assessment as proof of income — see CDCP eligibility for details.

Common questions about forms and paperwork.

Yes — request the digital intake form when you book your appointment. We email or text the link so you can complete it from home before arrival. If you prefer paper, that's available at the reception desk too.
Tell us — the reception team is used to helping with this. For specific medications, bring the bottles or a pharmacy printout. For conditions you're unsure about, leave the field and we'll discuss it with the dentist. We'd rather have an honest "not sure" than a wrong answer.
No — only the medical history update at each visit, which is short (2–3 minutes). The full intake is one-time. We update specific fields when you tell us something has changed.
Your information is governed by Ontario's Personal Health Information Protection Act (PHIPA). It's stored securely, accessed only by those involved in your care, and never sold or shared without your authorization. See our privacy page for details.
Yes. The owner's words: "We accept CDCP additionally our team will help those apply for it if they don't know how." Bring your income documentation (CRA Notice of Assessment from the most recent tax year) and we'll walk through the application. See CDCP eligibility for what to bring.
