Tooth pain has a small number of common causes, and each has a different fix. Sharp pain when biting often signals a cracked tooth, a high filling, or a fractured cusp — usually fixable with a crown or, for small cracks, a bonded restoration. Throbbing pain that keeps you up at night or radiates to the jaw and ear typically signals nerve involvement — usually an infected pulp needing a root canal or, if the tooth can't be saved, extraction. Sensitivity to cold that quickly settles after the stimulus is gone is often early decay or exposed dentin — usually fixable with a filling or a desensitizing protocol.

Cracked teeth deserve their own note. They're tricky because the crack often isn't visible on X-ray and the symptoms come and go. The classic pattern: sharp pain on release after biting ("rebound pain") on a specific tooth, sometimes with cold sensitivity. Diagnosis usually involves a bite test, a transilluminating light, and sometimes a dye stain. Catching a crack early — before it propagates into the nerve or splits the tooth — can save the tooth. Left too long, a cracked tooth often ends up needing root canal therapy, a crown, or extraction.
